BrainAge models can blur normal aging with Alzheimer's disease signals unless features are chosen for the diagnostic task.

Evidence

This modeling study ranked neuroimaging and neuropsychological features by age association and Alzheimer's-stage discrimination across five clinical groups, then tested BrainAge delta classification against direct logistic regression on the same features.

Caveat

The findings show a trade-off between age-prediction accuracy and Alzheimer's classification, so BrainAge delta is not automatically a reliable disease-specific biomarker.
