Understanding Aging and Heart Failure with Preserved Ejection Fraction

Updated

Abstract

Heart failure with preserved ejection fraction (HFpEF) is increasingly recognized as an age-predominant syndrome characterized by diastolic dysfunction despite preserved systolic performance.

  • Aging leads to a decline in the heart's ability to oxidize fatty acids, while glycolysis increases.
  • Impaired pyruvate oxidation restricts mitochondrial glucose oxidation, causing inefficient ATP production per oxygen molecule.
  • Mitochondrial deficits, including reduced biogenesis and increased oxidative stress, hinder bioenergetic reserve.
  • Chronic low-grade inflammation and protein misfolding contribute to myocardial fibrosis and increased ventricular stiffness.
  • Therapeutic strategies targeting metabolic and mitochondrial pathways show potential in ameliorating HFpEF-like conditions.
