Resistance patterns to ALK-targeted drugs in advanced lung cancer using genetic analysis from multiple centers

Abstract

Overall, 88 patients were enrolled, with various mutations identified in ALK TKI-resistant cancers.

  • 16% of crizotinib-resistant cancers showed ALK mutations, with specific mutations including L1196M and I1171T.
  • 22% of ceritinib-resistant cancers had ALK mutations, including G1128A and G1202R.
  • 17% of alectinib-resistant cancers exhibited ALK mutations, with G1202R being the most common.
  • 11% of lorlatinib-resistant cancers presented ALK mutations, notably G1202R/G1269A.
  • In paired tissue and cfDNA samples, 45% of cases had mutations identified in tissue, while 30% had mutations in cfDNA, indicating a concordance rate of 45%.
  • The study suggests that the mechanisms of ALK TKI resistance are heterogeneous and that compound ALK mutations may arise in lung cancers resistant to multiple treatments.
