Immune responses to PEG in mRNA lipid vaccines show changes across the population

Abstract

Vaccination with PEGylated mRNA-LNPs was associated with increased anti-PEG antibody levels.

  • Individuals vaccinated with PEGylated mRNA-LNPs showed qualitative changes in anti-PEG antibody binding behavior.
  • Anti-PEG IgG antibodies displayed progressively higher binding strength toward larger and more complex PEG-containing antigens.
  • Spikevax recipients exhibited increased avidity for both end-group and backbone PEG structures, as well as enhanced engagement with shorter PEG chains.
  • Polysorbate-containing or PEG-free vaccines did not produce similar alterations in antibody populations.
  • The emergence of high-avidity anti-PEG IgG could be linked to an increased risk of adverse events, including complement activation-related pseudoallergy and anaphylaxis.
