  • Early antidepressant treatments, including electroconvulsive therapy and monoamine inhibitors, faced safety and efficacy limitations.
  • The introduction of selective serotonin reuptake inhibitors improved tolerability but did not address all treatment needs.
  • N-methyl-D-aspartate (NMDA) receptor antagonists, like ketamine, showed fast-acting effects and shifted focus to glutamate modulation.
  • New therapeutic approaches include GABA modulation through neurosteroids and targeting the opioid system with kappa-opioid receptor antagonism.
  • Psychedelics, such as psilocybin, are being explored for their rapid and sustained antidepressant effects, though further research on safety is needed.
  • Challenges like diagnostic heterogeneity and the need for reliable biomarkers remain, despite advances in treatment strategies.
