Heart rate changes during tilt tests show nervous system imbalance in long COVID patients

Abstract

A total of 61 participants were enrolled, including 39 long COVID-19 patients who exhibited significant autonomic dysfunction during tilt table testing.

  • Long COVID-19 patients showed a significant increase in mean RR intervals during the upright phase (p = 0.0136).
  • Normalized low-frequency HRV was reduced (p = 0.0316) while normalized high-frequency HRV was increased (p = 0.0315) in long COVID-19 patients.
  • The low-frequency/high-frequency (LF/HF) ratio decreased (p = 0.0316), indicating a blunted sympathetic response.
  • Blood pressure responses were impaired, with attenuated changes in pulse pressure when transitioning from upright to recovery (p < 0.037).
  • Within-group analysis revealed persistent instability in RR intervals (p < 0.0001) and delayed recovery of pulse pressure after returning to a supine position.
