Cardiorespiratory fitness and risk of type 2 diabetes linked through biological aging in UK adults

Abstract

During 694,986 person-years of follow-up, the incidence rate of type 2 diabetes mellitus (T2D) was 3.78 per 1000 person-years.

  • Participants in the highest quartile of maximal oxygen uptake (VOmax) had a 56% lower risk of developing T2D compared to those in the lowest quartile.
  • Each standard deviation increase in VOmax was associated with a 28% lower risk of T2D.
  • Biological age acceleration mediated 8.2% of the association between VOmax and T2D risk, while phenotypic age acceleration mediated 9.1%.
  • Protective associations between VOmax and T2D risk were consistent across various subgroups including sex, age, ethnicity, and genetic risk.
  • VOmax exhibited strong inverse correlations with glucose, glycated hemoglobin, and triglycerides, and a positive correlation with high-density lipoprotein (HDL) cholesterol.
