Cell-free DNA changes in spinal fluid reflect disease biology in tau-related brain disorders

Updated

Abstract

Cerebrospinal fluid cfDNA concentrations were increased in Alzheimer's disease (AD) compared to other dementias.

  • Disease-associated methylation signatures were identified in cerebrospinal fluid (CSF) but were weak or absent in plasma.
  • AD exhibited widespread hypomethylation of inflammatory and senescence-related genes alongside hypermethylation of glymphatic and lymphatic regulators.
  • Behavioral variant frontotemporal dementia (bvFTD) showed intermediate changes in glial- and stress-related methylation.
  • Progressive supranuclear palsy (PSP) maintained relatively stable inflammatory and clearance-associated profiles.
  • Cerebrospinal fluid methylation panels consistently outperformed plasma-derived markers in classifying different tau-related dementias.
  • Receiver operating characteristic (ROC) analysis indicated strong discrimination for AD with AUC values between 0.84 and 0.85.
