Serum Neurofilament Light Levels May Help Distinguish Frontotemporal Dementia from Psychiatric Disorders, Depending on Age

Abstract

sNfL levels were 14.1 pg/mL in primary psychiatric disorders (PPD) compared to 44.1 pg/mL in behavioral variant frontotemporal dementia (bvFTD).

  • sNfL increased with age, with modest associations to sex, BMI, and diabetes status.
  • sNfL levels were higher in PPD compared to healthy subjects, but significantly lower than in bvFTD.
  • sNfL may differentiate PPD from bvFTD with an area under the curve (AUC) of 0.868 in individuals aged 40-60 years.
  • A cutoff of 16.0 pg/mL for sNfL was effective for distinguishing bvFTD from PPD in younger subjects.
  • No valid cutoff for distinguishing bvFTD from PPD was found for individuals over age 60, though specific sNfL values provided 90% diagnostic certainty.
