Effects of different blood sugar control levels in women with gestational diabetes

Abstract

Four randomized controlled trials involving 1731 women assessed the effects of tighter versus less-tight glycaemic control in gestational diabetes mellitus (GDM).

  • Tighter glycaemic control may be associated with a possible increase in hypertensive disorders of pregnancy.
  • There is likely little to no difference in caesarean section rates or induction of labour rates between tighter and less-tight glycaemic control.
  • No clear conclusions could be drawn regarding perinatal mortality, while tighter control likely does not influence the likelihood of infants being large-for-gestational-age.
  • Tighter glycaemic control may reduce adherence to treatment and significantly increases the use of pharmacological therapy.
  • The certainty of evidence for these outcomes ranges from low to moderate, primarily due to risk of bias and imprecision.
