Approximately 5% of individuals with epilepsy may experience transient worsening of seizures after mRNA vaccination.

  • Most individuals with epilepsy tolerate mRNA-based COVID-19 vaccination well.
  • Seizure exacerbation rates following vaccination are lower than those observed after COVID-19 infection.
  • Rare severe neurological events, such as status epilepticus, are noted but occur at rates comparable to or lower than natural background seizure incidence.
  • Potential mechanisms for seizure exacerbation include immune activation, neuroinflammation, and disruption of the blood-brain barrier.
  • Neurological side effects, such as Bell's palsy and transverse myelitis, may be more common with mRNA vaccines due to inflammation or cross-reactive immune responses.
  • Future research is needed to better understand individual risk factors and optimize vaccination approaches for individuals with epilepsy.
