Prescribing bias and negative outcomes of esketamine in major depression with substance use

Abstract

Esketamine users (n = 30,670) showed higher risks of comorbid substance use disorder across all categories compared to matched antidepressant and rTMS patients.

  • The highest relative risks for comorbid substance use disorder among esketamine users were for hallucinogen (RR 2.41), opioid (RR 2.26), and stimulant (RR 2.17) use when compared to the antidepressant group.
  • Compared to patients receiving repetitive transcranial magnetic stimulation (rTMS), esketamine users had an even higher risk for opioid use (RR 3.70).
  • Within the esketamine cohort, having a comorbid substance use disorder was associated with increased risks for self-harm (HR 2.99), suicide attempt (HR 2.13), emergency visits (HR 1.78), hospitalization (HR 1.70), and mortality (HR 1.12).
