Ethics approval and consent to participate: This study was conducted in Denmark, and was a health science survey questionnaire, and was not subject to medical research involving human biological material. According to Danish Legislation no registration details were therefore required [37]. In this study, participants were not subject to procedures and participated voluntarily. To ensure the anonymity of the participants no email addresses were collected, participants were issued a code number, and limited data were collected. It was therefore exempted from research ethics approval according to Danish legislation [37].
