Glucagon-Like Peptide-1 Receptor Agonists in People with Heart Failure and Reduced Heart Pumping

Essence

In patients with HFrEF, GLP-1 receptor agonist use was associated with lower 1-year mortality and fewer acute decompensated heart failure hospitalizations.

Evidence

This multicenter retrospective cohort study of adults with HFrEF used propensity score matching and found lower all-cause mortality (7.1% vs 10.2%; OR 0.68; matched HR 0.54) and lower ADHF risk (27.7% vs 32.8%; OR 0.79) in 1,275 GLP-1RA users versus 1,275 nonusers, with similar ACS, stroke/TIA, AF/flutter, and VT/VF events.

Caveat

Because this was an observational matched cohort rather than a randomized trial, residual confounding could explain part of the apparent benefit.
