Abstract
Ibogaine may reduce cravings and withdrawal symptoms in opioid and cocaine-dependent individuals but is associated with a clinically significant risk of cardiotoxicity.
- Observational and limited clinical trials suggest ibogaine and its metabolite noribogaine may help alleviate cravings and withdrawal symptoms.
- Ibogaine use is linked to QTc prolongation and potentially fatal heart rhythm disturbances, including Torsades des Pointes.
- These cardiotoxic events have been reported even at therapeutic doses in patients without existing heart conditions.
- Interindividual variability in the metabolism of ibogaine, particularly by the CYP2D6 enzyme, may increase the risk of cardiovascular issues.
- Current safety enhancements for ibogaine treatment involve monitoring cardiovascular health, using different dosing methods, and developing safer analogues.
