Immune-checkpoint inhibitors (ICIs) are now standard care for patients with locally advanced or metastatic non-small cell lung cancer.

  • ICIs can be used alone or in combination therapy for lung cancer patients lacking EGFR/ALK alterations.
  • Clinical trials are investigating the use of ICIs in early-stage lung cancer as either neoadjuvant or adjuvant therapy.
  • Current biomarkers like PD-L1 expression and tumor mutational burden are not fully reliable for patient selection.
  • A multidimensional immunogram may be necessary to better integrate predictive biomarkers for personalized immunotherapy.
  • Understanding the biological mechanisms of acquired resistance to ICIs is crucial for addressing treatment challenges.
