How irrigation affects clot-busting rtPA treatment in a brain bleed model

Abstract

Relative clot end weights after 60 minutes of irrigation varied significantly, showing a reduction to 46.3 ± 9.5% at 15 ml/h.

  • Irrigation rates of 15 ml/h resulted in the lowest relative end weight of clots after 60 minutes.
  • The combination of rtPA and irrigation did not provide additional clot weight reduction compared to rtPA alone.
  • Healthy and haemorrhagic cerebrospinal fluid showed no fibrinolytic activity under the tested conditions.
  • Plasminogen levels in clots decreased significantly after treatment with rtPA and high-rate irrigation.
  • Adding plasminogen to rtPA provided only marginal benefits for clot lysis.
