Abstract
A 35-year-old man achieved approximately 60 pounds of weight loss after initiating tirzepatide therapy for spinal epidural lipomatosis.
- Spinal epidural lipomatosis (SEL) is linked to obesity and can cause lumbar stenosis.
- The patient experienced severe symptoms, including neurogenic claudication and functional decline.
- MRI indicated multilevel lumbar SEL with severe spinal stenosis prior to treatment.
- After one year of tirzepatide therapy, the patient showed marked symptom resolution.
- Repeat MRI revealed near-complete regression of epidural fat, eliminating the need for surgery.
- Pharmacological weight loss may provide a nonoperative option for patients with obesity-related SEL.
