Approximately 15 million infants are affected by spontaneous preterm birth (sPTB) annually.

  • sPTB may reflect a state of accelerated gestational aging, where cellular stress pathways prematurely activate labor mechanisms.
  • Mitochondrial dysfunction and oxidative stress are linked to the onset of sPTB through impaired mitochondrial function and increased reactive oxygen species.
  • Cellular senescence pathways are activated in gestational tissues, promoting inflammation and extracellular matrix remodeling associated with premature labor.
  • Mechanistic pathways such as mitochondrial dynamics, NAD metabolism, and DNA damage responses interact to influence sPTB.
  • Emerging strategies involving biomarkers and targeted therapies, like mitochondrial antioxidants and inflammasome inhibitors, are proposed to address sPTB.
