Brain chemical changes linked to symptoms in metachromatic leukodystrophy using MRI analysis without preset models

Abstract

MLD spectra from 29 patients exhibited pronounced differences compared to 12 controls, particularly in white matter regions.

  • Differences in MLD spectra were most significant in white matter, with increased levels of myo-inositol, choline, and aspartate, and decreased levels of N-acetylaspartate (NAA) and glutamine.
  • Late-infantile patients showed more severe metabolic changes than juvenile patients, especially in metabolites linked to cognitive and motor function.
  • A strong negative correlation was observed between NAA levels in the corticospinal tract and motor function, with a correlation coefficient of -0.75 (p < 0.001).
  • Cognitive function, measured as IQ, correlated highly with NAA levels in frontal white matter (r = 0.84, p < 0.001).
  • A post-hoc analysis indicated a negative correlation between NAA levels in urine and the NAA peak interval from MRS data (r = -0.584, p < 0.001).
