Widespread problems with energy production in muscle cells in inclusion body myositis

Abstract

In a study of 67 patients with inclusion body myositis (IBM), mitochondrial respiration was found to be significantly impaired compared to 45 controls.

  • IBM samples showed reduced maximal mitochondrial respiration per tissue weight in both State 3 and uncoupled states.
  • Male patients had decreased intrinsic respiration in State 3 after adjusting for mitochondrial content, while female patients exhibited greater intrinsic respiration under leak states.
  • Complex I activity was notably decreased in female patients, with a positive correlation between complex II control ratio and disease duration and severity.
  • Lower RNA levels of all mitochondrial complexes and reduced protein expression of complexes I, III, IV, and V were observed in IBM samples.
  • IBM samples displayed lower maximal hydrogen peroxide emission alongside a higher total antioxidant capacity, which correlated positively with disease duration in female patients.
  • Impaired mitochondrial respiration, altered mitophagy, and reduced mitochondrial DNA content may be interconnected in IBM and hold clinical relevance.
