Abstract

Two distinct subtypes of Parkinson's disease with mild cognitive impairment (PD-MCI) were identified among 104 drug-naïve patients.

  • Subtype 1 includes 63 patients with above-normal gray matter volume in specific brain regions, indicating localized brain changes.
  • This subtype shows signs of microstructural disruption and is associated with executive dysfunction and metabolic dysfunction pathways.
  • Subtype 2 consists of 41 patients with below-normal gray matter volume across broader brain areas, yet maintains microstructural integrity.
  • Cognitive network connectivity in this subtype is enhanced, suggesting effective compensation despite volumetric loss.
  • Distinct molecular signatures related to cellular organization and signaling were found in both subtypes.
  • The two-subtype structure was validated in an independent cohort, reinforcing the observed neuroanatomical and cognitive differences.
