Using Price Comparisons and Drug Effectiveness to Predict First Medicare Drug Price Offers for 2027

Estimated discounts for the 15 drugs selected for the Medicare Drug Price Negotiation Program range from 32% to 78% off list price.

  • Six drugs had initial offers anchored by statutory or Big 4/Federal Supply Schedule prices.
  • Four drugs showed approximately 20% reductions from net price based on clinically comparable branded alternatives.
  • Two drugs with primarily generic alternatives received larger discounts of around 30%.
  • Three drugs were assigned established maximum fair prices or market-based premiums from previous negotiations.
  • Estimated discounts varied from 16% to 56% off net price across all drugs.
