Muscle control differences in infant and adult Pompe patients revealed by combined molecular and structural analysis

Updated

Abstract

A multi-omic analysis of skeletal muscle samples from 37 patients with Pompe disease revealed distinct regulatory patterns between late onset and infantile-onset forms.

  • Deregulated muscle processes were identified in late onset Pompe disease (LOPD) and infantile-onset Pompe disease (IOPD) samples, with differences in regulation at both transcript and protein levels.
  • Transcriptomic profiling indicated that immune processes were upregulated in both subtypes, while metabolic processes were primarily downregulated in LOPD.
  • A decrease in muscle development-related transcripts was specifically noted in IOPD samples.
  • Mass spectrometry analysis found reduced levels of sarcomeric proteins in LOPD, with alterations in chaperone-assisted selective autophagy observed in both subtypes.
  • Proteins involved in vesicle trafficking and biosynthesis showed a correlation with muscle pathology severity in LOPD.
  • Enzyme replacement therapy (ERT) in IOPD patients normalized many deregulated transcripts, although not entirely to baseline levels, and increased proteins associated with mitochondrial function and sarcomeric activity.
