A total of 78 studies were reviewed to synthesize evidence on SARS-CoV-2's potential role in post-acute sudden cardiac death and pulmonary thromboembolism.

  • SARS-CoV-2 may persist in cardiovascular and other tissues months after initial infection, detected through viral RNA and antigens.
  • Viral entry into heart and blood vessel cells is facilitated by ACE2, with cathepsin L compensating for low TMPRSS2 levels.
  • Mechanisms like endothelial dysfunction and thromboinflammation are associated with heart rhythm disorders and blood clots, though definitive causal links are unclear.
  • Population studies indicate sustained higher rates of cardiovascular deaths and pulmonary embolism risk, particularly in unvaccinated individuals, after acute SARS-CoV-2 infection.
