How Starting Order of Two Diabetes Drugs Affects Kidney Function in Type 2 Diabetes

Abstract

Among 14,774 matched patients, the combination of SGLT2 inhibitors and GLP-1 receptor agonists is associated with a smaller decline in kidney function compared to GLP-1 receptor agonist monotherapy.

  • The mean decline in kidney function (measured as eGFR) for the combination group was -5.4 mL/min/1.73 m² at 1 year and -6.1 mL/min/1.73 m² at 2 years.
  • Compared to GLP-1 RA monotherapy, the combination therapy showed a difference in eGFR reduction of 2.2 mL/min/1.73 m² at 1 year and 2.1 mL/min/1.73 m² at 2 years, both statistically significant (p < 0.001).
  • No significant difference in eGFR decline was observed when comparing the combination therapy to SGLT2 inhibitor monotherapy.
