  • Comparative analysis identified donanemab as having the highest incidence of amyloid-related imaging abnormalities with hemorrhage (ARIA-H).
  • Five specific determinants were linked to variations in ARIA-H risk among monoclonal antibodies.
  • Enhanced clearance of mature amyloid plaques was associated with increased ARIA-H risk.
  • A reduced capacity to bind small oligomers predicted a higher incidence of ARIA-H.
  • N-terminal-targeting antibodies exhibited greater ARIA-H incidence compared to mid/C-terminal binders.
  • IgG4-based monoclonal antibodies showed a higher incidence of ARIA-H than IgG1 constructs.
