Very Low Energy Ketogenic Diet Reduces Liver Fat More Than Mediterranean Diet in MASLD: Results from a Small Randomized Study

Abstract

The very low energy ketogenic diet (VLED) led to a 77% relative reduction in hepatic steatosis compared to 14% with the Mediterranean diet (MD).

  • Participants on the VLED experienced a total body weight loss of -13% at week 12, while the MD group had a loss of -4%.
  • At 24 weeks, the VLED group maintained a weight loss of -14%, compared to -3% in the MD group.
  • Both diets improved liver histology, but the VLED group showed a greater reduction in the NAFLD activity score (VLED: -2 vs MD: -1).
  • The VLED was associated with a significant improvement in hepatic steatosis and weight loss compared to the MD in individuals with overweight or obesity and MASLD.
