Blocking ALK Signals Reverses Reduction of Iodine Transporter and Restores Radioiodine Uptake in Thyroid Cells Caused by STRN-ALK

Abstract

A fourfold decrease in RAI uptake is associated with the loss of sodium-iodine symporter (NIS) in ALK fusion-driven thyroid cancer.

  • Downregulation of NIS occurs early in the progression of ALK-driven thyroid cancer, even in well-differentiated stages.
  • A complete loss of NIS expression is observed in poorly differentiated thyroid carcinomas.
  • Acute expression of STRN-ALK in thyroid cells is linked to increased signaling outputs in multiple pathways, leading to downregulation of thyroid differentiation and iodine metabolism genes.
  • Inhibition of ALK with FDA-approved inhibitors can reverse the loss of NIS and restore RAI uptake.
  • Treatment with the MEK inhibitor selumetinib and the JAK1/2 inhibitor ruxolitinib shows a cumulative effect in restoring NIS expression.
