Do Oncology Value Frameworks Measure Absolute or Relative Survival Benefits? Analysis of Clinical Trials

Abstract

ESMO preliminary magnitude of clinical benefit grades (PMCBGs) showed low to moderate correlations (ρ = 0.44) with the restricted mean survival time (RMST) difference.

  • The correlation between ESMO-PMCBGs and median survival difference was moderate to high (ρ = 0.64).
  • ASCO clinical benefit scores (CBSs) demonstrated low to moderate correlations with RMST difference (ρ = 0.43) and median survival difference (ρ = 0.44).
  • Compared to hazard ratios (HRs), ESMO-PMCBGs had a low correlation (ρ = 0.47), while ASCO-CBSs had a higher correlation (ρ = 0.76).
  • Neither framework consistently served as an absolute measure of survival benefit.
  • Incorporating a direct measure of absolute clinical benefit, such as RMST difference, into these frameworks may enhance their evaluation of survival efficacy.
