Bile Duct and Pancreas Safety of Incretin-Based Treatments in Obesity and Type 2 Diabetes Compared Between Tirzepatide, Semaglutide, and DPP-4 Inhibitors

Updated

Abstract

After matching, 281,283 patients were included in each GLP-1 receptor agonist and DPP-4 inhibitor cohort.

  • GLP-1 receptor agonist therapy was associated with lower risks of cholelithiasis, cholecystitis, cholangitis, acute pancreatitis, and cholecystectomy compared to dipeptidyl peptidase-4 inhibitors.
  • The risk of cholelithiasis was 1.02% for GLP-1 receptor agonists versus 1.16% for DPP-4 inhibitors.
  • In head-to-head comparisons, semaglutide was associated with higher risks of cholelithiasis and cholecystitis compared to tirzepatide.
  • Risks of pancreatitis, cholangitis, and cholecystectomy did not differ significantly between semaglutide and tirzepatide.
  • The findings suggest a more favorable biliary safety profile for tirzepatide relative to semaglutide.
