Bipolar depression is associated with abnormalities in multiple biological systems over time.

  • More advanced bipolar disorder may exhibit disruptions in inflammatory signaling and neurotrophic regulation.
  • Structural and functional brain imaging abnormalities are often linked to the current mood state rather than cumulative illness progression.
  • Candidate biomarkers, such as peripheral BDNF and inflammatory markers, show limited specificity in reflecting disease progression.
  • Ketamine and psychedelic compounds have been shown to influence some of the same biological pathways observed in bipolar disorder.
  • Preliminary studies suggest potential antidepressant effects of psilocybin and 5-MeO-DMT in bipolar II depression under controlled conditions.
  • Current findings do not establish a reliable biomarker framework for staging or neuroprogression in bipolar disorder.
