How Twin Placenta Type and Timing of One Baby's Loss Affect Early Birth Risk in Twin Pregnancies

Abstract

The median gestational age at birth for pregnancies complicated by single intrauterine death (sIUD) was 32.0 weeks, compared to 36.7 weeks for those without fetal loss.

  • Pregnancies affected by sIUD had a higher risk of preterm birth (PTB) at < 34 weeks (relative risk, 4.3), < 32 weeks (6.1), and < 28 weeks (12.4) compared to those without fetal loss.
  • This increased risk of PTB was observed in both monochorionic (MC) and dichorionic (DC) twin gestations.
  • In MC twins, the risk of PTB was not significantly higher than in DC pregnancies for gestational ages before 34, 32, or 28 weeks.
  • The risk of PTB at < 34 weeks was greater when sIUD occurred later in the pregnancy.
