A total of 241 trials involving 31,452 participants indicated that cognitive restructuring may significantly enhance treatment efficacy for chronic insomnia disorder.

  • Cognitive restructuring is associated with a remission incremental odds ratio of 1.68, suggesting it plays a critical role in improving outcomes.
  • Third-wave components and sleep restriction are linked to remission incremental odds ratios of 1.49 each, indicating their potential importance in treatment.
  • In-person therapist-led programs show the highest benefit with an odds ratio of 1.83, suggesting they may be the most effective delivery method.
  • Sleep hygiene education does not appear essential for treatment efficacy, with an odds ratio of 1.01.
  • Relaxation procedures may have a counterproductive effect, indicated by an odds ratio of 0.81.
  • The most effective combination involves cognitive restructuring, third-wave components, sleep restriction, and stimulus control delivered in-person, increasing remission rates significantly.
