GLP-1 receptor agonists (GLP-1RAs) are associated with substantial and durable weight loss but have common gastrointestinal side effects.

  • Gastrointestinal adverse effects, including nausea and constipation, frequently lead to discontinuation of GLP-1RAs.
  • Weight regain is common after stopping GLP-1RA treatment, highlighting the chronic nature of obesity.
  • Dietary fibers engage the gut-brain axis and stimulate the body's own GLP-1 production through microbial fermentation.
  • Fiber can strengthen gut barrier function and promote an anti-inflammatory immune response, though weight loss with fiber is typically more modest than with GLP-1RAs.
  • Tolerability of fiber may be limited by bloating or gas, especially with rapid increases in intake.
  • A framework is suggested to prioritize dietary fiber and lifestyle changes in treatment while reserving GLP-1RA therapy for high-risk individuals.
