The glymphatic system is a unique brain fluid pathway that primarily operates during sleep.

  • The brain relies on cerebrospinal fluid (CSF) for maintaining a stable environment for neuronal function due to limited transcapillary filtration.
  • Astrocytic vascular endfeet create perivascular spaces that facilitate the flow of CSF into the brain.
  • Glymphatic clearance of metabolic waste, including amyloid-β, is most active during sleep.
  • Physiological processes such as the cardiac cycle, respiration, and slow vasomotion enhance CSF inflow along periarterial spaces during sleep.
  • The brain's extracellular space contains proteoglycans and hyaluronan, allowing it to rapidly expand and shrink in response to the sleep-wake cycle.
