A 63-year-old man exhibited progressive pustular folliculitis on his forearms and lower leg, despite traditionally limited scalp involvement.

  • Folliculitis decalvans is typically confined to the scalp but may also manifest on other body parts, as demonstrated in this case.
  • The patient had a 3-year history of inflammatory patches and scarring alopecia on his limbs, rather than the scalp.
  • Isolation of Staphylococcus aureus and specific histopathological findings supported a diagnosis of folliculitis decalvans-like pustular folliculitis.
  • Biopsies showed features such as follicular pustules and fibrosis, which are characteristic of folliculitis decalvans.
  • Treatment with antibiotics and topical corticosteroids was effective, similar to responses observed in scalp cases of folliculitis decalvans.
