Weight loss and blood sugar improvement in type 2 diabetes: formula diet versus diabetes drugs

Updated

Abstract

Essence

This review suggests -based lifestyle interventions may produce greater weight loss than several antidiabetic drug strategies in obese patients with type 2 diabetes, with similar long-term HbA1c reduction.

Evidence

This systematic review compared 3 formula diet-based lifestyle interventions with 47 randomized placebo-controlled pharmacologic studies of GLP-1-based therapies and SGLT-2 inhibitors, covering 54 articles and 87,871 patients using intention-to-treat estimated treatment differences for weight and HbA1c.

Caveat

The comparison is indirect across separate study sets rather than head-to-head trials, so apparent weight and glycemic differences may reflect between-study heterogeneity.

What this is

  • This systematic review compares -based lifestyle interventions and pharmacological treatments for type 2 diabetes.
  • It includes 54 studies with 87,871 patients, focusing on weight loss and HbA1c reduction.
  • The findings suggest that formula diets may lead to greater weight loss compared to pharmacological therapies, with similar long-term glycemic control.

Essence

  • -based lifestyle interventions may achieve greater weight loss than pharmacological treatments for type 2 diabetes, while maintaining comparable long-term glycemic control.

Key takeaways

  • -based interventions led to a weight reduction of -5.6 kg after less than 12 months, compared to -2.6 kg for pharmacological interventions.
  • After 12 months or more, formula diets resulted in a weight loss of -7.3 kg, whereas pharmacological treatments achieved -3.1 kg.
  • Long-term HbA1c reduction was similar between both interventions, with a reduction of -0.7% for both and pharmacological therapies.

Caveats

  • The review lacks long-term studies for some treatments, particularly tirzepatide, limiting the understanding of its efficacy beyond 12 months.
  • Differences in baseline characteristics, such as BMI and HbA1c levels, may affect the interpretation of treatment effects.
  • The heterogeneity of included studies prevents a meta-analysis, which could have provided more robust conclusions.

Definitions

  • Formula diet: Meal replacements that substitute main meals with prepackaged, nutritionally complete products designed for weight loss.
