Genetic Links to Benefits and Risks of Blood Sugar Drugs on Heart, Kidney, and Metabolic Health

Abstract

Genetic proxies for sodium-glucose cotransporter 2 inhibitors (SGLT2i) are associated with reduced risks of coronary artery disease, heart failure, and metabolic syndrome.

  • SGLT2 inhibitors are linked to a lower risk of coronary artery disease (OR 0.85) and heart failure (OR 0.66).
  • GLP-1 receptor agonists are associated with decreased risks of coronary artery disease (OR 0.90) and chronic kidney disease (OR 0.87).
  • Metformin shows a protective association with coronary artery disease (OR 0.51) and peripheral artery disease (OR 0.99).
  • Sulfonylureas are modestly associated with reduced coronary artery disease risk (OR 0.98).
  • Insulin is related to a higher risk of metabolic syndrome (OR 1.07), while thiazolidinediones are linked to increased heart failure risk (OR 1.12).
  • Supplementary analyses support protective associations for several cardiovascular outcomes and highlight potential risks associated with insulin and thiazolidinediones.
