Weight gain during pregnancy and birth outcomes in women with long-term high blood pressure

Abstract

In a cohort of 101,259 women with chronic hypertension, weight gain of ≥20 lb over National Academy of Medicine guidelines is associated with several adverse outcomes.

  • Hypertensive women gaining ≥20 lb over guidelines have almost double the odds of eclampsia compared to those within guidelines.
  • These women are also 60% more likely to require a cesarean delivery.
  • Excess weight gain is associated with higher odds of 5-minute Apgar scores below 7 and neonatal intensive care unit admissions.
  • Gaining ≥20 lb is linked to significantly increased odds of having large-for-gestational-age neonates.
  • Conversely, women gaining less than the guidelines show increased odds of small-for-gestational-age neonates.
