Different effects of GLP-1 receptor drugs in heart failure with preserved versus reduced pumping ability

Abstract

GLP-1 receptor agonists significantly reduced heart failure worsening events in patients with preserved ejection fraction (HFpEF) (HR 0.67, 95% CI 0.50-0.89).

  • In HFpEF, GLP-1 receptor agonists improved NT-proBNP levels, six-minute walk distance, Kansas City Cardiomyopathy Questionnaire scores, and systolic blood pressure.
  • The composite outcome of heart failure worsening events or cardiovascular death was also reduced in HFpEF patients (HR 0.76, 95% CI 0.64-0.90).
  • No benefit was observed for heart failure worsening events in patients with reduced ejection fraction (HFrEF) (HR 1.23, 95% CI 0.89-1.69).
  • Gastrointestinal adverse events were the most common reason for discontinuation of GLP-1 receptor agonists (RR 3.12, 95% CI 1.28-7.63).
  • Results indicate the need for careful patient selection to optimize adherence due to gastrointestinal intolerance.
