Blood markers of inflammation and brain injury vary with COVID-19 severity in people with lasting neurological symptoms

Abstract

Among 265 participants with long COVID, individuals with severe and critical disease showed significantly higher concentrations of multiple inflammatory and neuroinjury-related biomarkers.

  • Patients with severe and critical COVID-19 had higher levels of IL-6, IL-1β, TNF-α, and IL-8 compared to those with mild disease.
  • Elevated levels of neuroinflammatory and neurodegenerative biomarkers, such as neurofilament light chain (NfL), glial fibrillary acidic protein (GFAP), and phosphorylated tau181, were found in patients with severe illness.
  • A reduction in the Aβ42/40 ratio was noted in individuals with severe and critical disease.
  • Strong positive correlations between neurodegenerative biomarkers and pro-inflammatory cytokines were observed in critically ill patients, while such associations were weak or absent in milder cases.
  • Older age was linked to greater disease severity and a higher risk of persistent neurological complications.
