Paxlovid didn't prevent long COVID in a randomized trial. Now what?

Long COVID research had a big week — and not all the news was good.

A high-profile Norwegian trial just tested one of the most promising prevention strategies and came up short.

  • The PANORAMIC Norway trial was a double-blind, placebo-controlled study testing whether nirmatrelvir-ritonavir (Paxlovid) during acute COVID-19 infection could prevent long COVID — specifically the fatigue, cognitive problems, and breathlessness that linger for months.
  • Cohort studies had suggested antivirals might cut long COVID risk, which made this prospective test the one researchers were waiting for.

Implications

The Paxlovid trial failure sharpens an uncomfortable question: if the most available antiviral doesn't prevent long COVID under controlled conditions, what does? Biomarker and dietary data point toward inflammation as a key driver — but whether intervening on inflammation during or after acute infection actually changes long-term outcomes remains untested.
