Abstract

A total of 30 randomized controlled trials involving 12,461 participants assessed the effectiveness of a Mediterranean-style diet on cardiovascular disease (CVD).

  • Primary prevention trials did not report on clinical endpoints such as CVD mortality or non-fatal events.
  • There is low-quality evidence indicating a possible small reduction in total cholesterol and moderate-quality evidence for reductions in systolic and diastolic blood pressure.
  • The PREDIMED trial showed a reduction in strokes associated with the Mediterranean diet intervention compared to a low-fat diet.
  • For secondary prevention, low-quality evidence suggests a reduction in CVD mortality and total mortality in CHD patients following a Mediterranean diet.
  • Overall, the evidence for the Mediterranean diet's benefits on clinical endpoints and CVD risk factors remains uncertain.
