Abstract

Essence

Metabolic surgery was associated with lower long-term mortality, cardiovascular events, nephropathy, and retinopathy than GLP-1 receptor agonist treatment in diabetes with obesity.

Evidence

This adjusted comparative cohort analysis followed 1,657 surgery patients and 2,275 GLP-1 RA patients with type 2 diabetes and obesity for a median 5.9 years.

Caveat

Because treatment was not randomized and newer, more weight-reducing GLP-1 RAs were not the comparator, residual confounding and current-drug generalizability remain boundaries.
