Four-year survival and quality of life after intensive care for severe COVID-19 lung failure

Abstract

Cumulative 4-year mortality among ICU-treated COVID-19 ARDS patients is 45%.

  • Thirty-day mortality was 29.0%, indicating significant early mortality risk.
  • Older age and higher white blood cell count at ICU admission were linked to increased mortality.
  • Among survivors, 27.5% experienced clinically relevant fatigue and 46.8% reported insomnia.
  • A notable portion of survivors had persistent limitations in functional status and quality of life.
  • Rehabilitation was reported by 39% of survivors and was associated with lower quality-adjusted life years.
  • Median 4-year quality-adjusted life years (QALY) was 3.7, influenced by fatigue, dyspnoea, and cognitive issues.

The study comprised two components with different consent requirements. For the retrospective ICU-phase component, anonymized clinical and laboratory data were extracted from routine electronic medical records under an ethics-approved waiver of written informed consent, in accordance with applicable Polish regulations and GDPR provisions for research using de-identified retrospective data. The prospective component consisted of structured telephone follow-up conducted four years after ICU admission; for this part, verbal informed consent was obtained from all interview participants prior to data collection, as approved in the study protocol.
