Heart Attack Without Major Artery Blockage After Large Weight Loss During Tirzepatide Treatment: A Case Report

Updated

Abstract

A 48-year-old man achieved 30 kg weight loss after 18 months on tirzepatide but presented with a non-ST elevation myocardial infarction (NSTEMI).

  • Tirzepatide, a dual GIP and GLP-1 receptor agonist, is associated with substantial weight loss and improved cardiometabolic risk factors.
  • No established association exists between licensed tirzepatide therapy and acute coronary syndrome (ACS).
  • The case involved a patient with a history of obesity, prediabetes, smoking, and familial coronary artery disease who developed chest pain and dyspnoea.
  • Diagnostic tests indicated ischaemic changes and confirmed NSTEMI, alongside severe triple-vessel coronary artery disease.
  • Significant weight loss may have uncovered previously undetected coronary artery disease due to increased physical activity.
  • This report emphasizes the need for careful evaluation of new exertional chest pain in patients on tirzepatide, even with improvements in weight and metabolic health.
