Pain management guidelines for critically ill patients near the end of life: a systematic review

Updated

Abstract

Ten publications on pain management at the end-of-life in the ICU were evaluated.

  • Low quality scores were noted in various domains, particularly in applicability and development rigor.
  • Recommendations consistently endorsed the use of a quantitative tool for pain assessment.
  • Narcotics for pain relief and benzodiazepines for anxiety relief were recommended.
  • There was a consensus against prescribing neuromuscular blockers during life support withdrawal to assess pain.
  • High doses of opioids and sedatives for pain control were supported, even with the risk of hastening death.
  • Quality indicators were suggested to enhance pain management in the ICU for dying patients.
