Long COVID: Different Symptom Types, How It Changes Over Time, and Key Factors That May Predict It

Abstract

Of 627 patients hospitalized with acute COVID-19, 252 (40.2%) met the criteria for long COVID.

  • Comorbidity burden is the strongest predictor of long COVID, with patients having a single condition showing a 4-fold increase in odds.
  • Older age, acute disease severity, and elevated admission fibrinogen are also significant predictors of developing long COVID.
  • The risk of developing long COVID declined significantly from the Wild-type/Alpha variants (>50%) to the Delta/Omicron variants (<21%).
  • A distinct phenotypic shift was observed, with systemic fatigue increasing from 7.0% to 32.4% and memory difficulties emerging in the post-acute phase.
  • Latent Class Analysis identified two clinical phenotypes: fatigue-dominant and multisystem.
