Protecting Muscle Health During Weight Loss with Lifestyle, Surgery, and Medication

Updated

Abstract

Caloric restriction is associated with substantial reductions in body weight and fat mass, but it may also lead to variable losses of fat-free and lean mass.

  • Higher protein intake may support the preservation of lean tissue during energy restriction, especially when combined with resistance exercise.
  • Resistance training is consistently linked to the preservation of lean mass and improvements in muscle strength and physical function.
  • Aerobic exercise may provide complementary benefits for cardiorespiratory fitness and walking capacity.
  • Bariatric surgery and glucagon-like peptide-1 receptor agonist therapy can lead to significant weight loss, yet reductions in lean mass are also observed.
  • Reductions in lean mass do not always correspond to clinically meaningful impairments in muscle strength or physical performance.
  • An individualized approach with adequate protein intake, energy restriction, and structured exercise may help mitigate adverse changes in muscle health during obesity treatment.
